INTRA URETHRAL UTTARA BASTI

INTRODUCTION

The procedure in which the medicine is administered through vesicular / urethral route.

MATERIALS REQUIRED
  • Ref General Requirements SOP
  • Basti netra – Red rubber catheter no.8
  • Basti putaka – 20 cc to 50 cc Syringe.
  • Medicine – Oil/ Ghee/ Kashaya/ Ksheerapaka
  • Xylocaine Gel
  • Normal saline
  • Sponge holding forceps – 1
  • Surgical gloves – 2
  • Hole towel – 1
  • Gauze pieces – 4
  • Cotton
  • Bowl – 1
  • Kidney Tray – 1

PREPARATION

 

  1. Preparation of tray
  • Take a clean tray.
  • Put medicine and all other required materials for sterilization in this tray.
  1. Preparation of patient
  • Check vitals.

PRADHANA KARMA

Female

  • The patient is asked to lie in lithotomy position.
  • The genital area is cleaned from inner to outer vestibule to both thigh region with normal saline using sponge holding forceps.
  • The hole towel is placed exposing the genital region.
  • The tip of red rubber catheter is lubricated with xylocaine gel up to 5-6 cm.
  • The catheter is then gently inserted into the urethral meatus up to 5 cm.
  • The residual urine is collected in a kidney tray.
  • The syringe filled with medicine is then attached to the distal end of the catheter and the medicine is introduced slowly and continuously into the urinary bladder.
  • Once the administration of medicine has been completed, the catheter is gently removed.

Male

  • The patient is asked to lie in lithotomy position.
  • The genitals and surrounding areas are washed with normal saline using sponge holding forceps. Then, the prepuce is retracted and the glans penis is washed thoroughly.
  • The hole towel is placed exposing the penis.
  • The tip of red rubber catheter is lubricated with xylocaine gel.
  • The catheter is then gently inserted into the urethra.
  • The residual urine is collected in a kidney tray.
  • The syringe filled with medicine is then attached to the distal end of the catheter and the medicine is introduced slowly and continuously into the urinary bladder.
  • Once the administration of medicine has been completed, the catheter is gently removed.
PASCHAT KARMA
  • The patient is asked to lie down in supine position.
  • Patient is advised to void urine whenever urge comes.

  • 30 – 45 minutes

PRECAUTIONS
  • Uttarabasti procedure should be carried out by an expert physician under all aseptic precautions.
  • The medicine for uttarabasti along with all the required materials should be autoclaved.
  • Basti netra should be insert carefully without shaking.
  • Basti Netra should not be inserted beyond and lesser than the prescribed limit.
  • Urge of urine or motion neither be ignored nor forced.
  • Avoid spicy food and any kind of sexual activity.
COMPLICATIONS AND MANAGEMENT
  • Lower abdominal pain: it is usually self-limiting and if necessary give fomentation with hot water bag and antispasmodics can be given.
  • Infection: urethral douche with triphala kashya. This condition may require a course of antibiotic therapy.
  • Retention of basti dravya: probe is inserted in mutramarga and abdomen is pressed forcefully below the umbilicus, insert sodhana dravya siddha phala varti in mutramarga.
  • Burning sensation in the urinary bladder: administer a urethral enema containing Yaṣhṭimadhu seeta kashaya with sarkara and madhu .
