Abhyanga

INTRODUCTION

Abhyanga is the procedure in which massage is performed on the body using warm medicated oil by different stroke patterns for a specific duration of time.

PREPARATION

Preparation of Thalam

Preparation of tray

 

  • Take a clean tray.
  • Put medicated oil for abhyanga in a vessel, langotti, towel, thalam.

Preparation of patient

  • Check vitals.
  • Jeernaharalakshana assessed.
  • Vegotsarga
  • Make patient wear langotti.
PURVA KARMA
  • Patient is asked to sit comfortably on a knee height chair.
  • Head of the patient is cleaned with a towel.
PRADHANA KARMA
  • Abhyanga is done in 7 postures namely Sitting, Supine, Right Lateral, Supine, Left Lateral, Supine and Sitting.
  • Sufficient quantity of oil needed for shiro abhyanga is taken and uniformly applied over the head.
  • Mild massage is performed covering the entire head.
  • Then the oil is applied over both ears and massaged uniformly.
  • The vertex of the patient is marked by positioning the distance from tip of the nose to vertex using thumb and middle finger of the patient .
  • Thalam is applied on the vertex of the patient.
  • Sitting Posture
  • Patient is made to sit in dhroni with legs extended and both palms resting on the knees.
  • Whole body and foot of the patient is cleaned using a towel.
  • Taila for abhyanga is made lukewarm using a hot water bath.
  • Temperature of the taila is checked by pouring it over the dorsum of hand.
  • The warm oil is massaged over the palms and soles with linear strokes.
  • Then lukewarm taila is then applied on either side of the patient’s body , low back and the legs uniformly .
  • Synchronised massage with linear strokes is done on the anterior part of the hand followed by massage on the posterior aspect of the hand  covering the abdomen .
  • Always pressure is applied towards  downward direction (along the direction of body hairs).
  • Then massage in linear strokes is done covering the anterior aspect of both legs in a synchronised manner .
  • Massage is done alternatively in linear pattern covering the chest, shoulder and hand region.
  • Massage in linear strokes is done covering both the sides of neck .
  • Supine Posture
  • The patient is asked to lie in supine posture.
  • Gentle massage is done on the face by one therapist using warm medicated oil .
  • Linear stroke massage is done starting from abdomen to hand in synchronised manner.
  • Alternate linear massage is done over the abdomen covering the hand region .
  • Both the shoulder joints, elbow joints and wrist joint are massaged in a circular pattern .
  • The fingers are massaged uniformly
  • Synchronized massage is done covering anterior aspect of thighs and legs in linear strokes.
  • Whole body synchronized massage is done in linear strokes from hand to foot covering the abdomen 
  • The hands are raised and massaged with linear strokes covering abdomen and posterior aspect of hand.
  • Whole body linear stroke massage is done from hand to foot in hand raised position .
  • Cupping stroke massage is done over the chest and hand region
  • Cupping stroke massage is also done in thighs and leg region .
  • Linear finger tapping stroke massage is done over chest and hand region
  • Finger tapping massage is also done in thighs and leg region .
  • Squeezing compression stroke massage is done over chest and hand region
  • Squeezing compression stroke massage is done over thighs and leg region
  • After different stroke massage linear massage is done on the whole body from hand to foot covering the abdomen .
  • Left Lateral Posture
  • The patient is asked to lie on the left side of his body.
  • Right side of the trunk, right arm and lateral aspect of the right leg is covered in this posture.
  • Linear massage is done from hand to hip and  from hip to toe by therapist on the left side of the patient .
  • Linear stroke massage is done from whole of the back including spine, hip to toe by the therapist on the right side .  
  • Linear stroke massage is done covering the lateral aspect of the both legs .

Supine Posture

  • The patient is asked to lie in supine posture.
  • Linear stroke massage is done starting from abdomen to hand in synchronised manner.
  • Alternate linear massage is done over the abdomen covering the hand region .
  • Synchronized massage is done covering anterior aspect of thighs and legs in linear strokes.
  • Whole body synchronized massage is done in linear strokes from hand to foot covering the abdomen 
  • Right Lateral Posture
  • The patient is asked to lie on the right side of his body.
  • Left side of the trunk, left arm and lateral aspect of the left leg is covered in this posture.
  • Linear massage is done from hand to hip and from hip to toe by therapist on the right side of the patient .
  • Linear stroke massage is done from whole of the back including spine, hip to toe by the therapist on the left side . 
  • Linear stroke massage is done covering the lateral aspect of the both legs .

Supine Posture

  • The patient is asked to lie in supine posture.
  • Linear stroke massage is done starting from abdomen to hand in synchronised manner.
  • Alternate linear massage is done over the abdomen covering the hand region .
  • Synchronized massage is done covering anterior aspect of thighs and legs in linear strokes.
  • Whole body synchronized massage is done in linear strokes from hand to foot covering the abdomen 
  • Sitting Posture

PASCHAT KARMA
  • Oil is wiped from sole of the foot to avoid slipping.
  • Thalam is removed.
  • The patient is advised to take steam bath if needed
  • Patient is asked to take hot shower after resting for a while .
  • Rasnadi choorna is applied over vertex after bath.

CONTRAINDICATIONS
  • Acute fever
  • Indigestion
  • Haemorrhagic disorders
  • Diarrhoea
PRECAUTIONS
  • Avoid excess pressure in tender areas.
  • While massaging, only gentle strokes should be applied.
  • Care should be taken to avoid burns during the procedure by hot oil.
  • After the procedure, the sole is wiped well to avoid slipping.
  • Synchronous massage should be maintained throughout the procedure.
COMPLICATIONS AND MANAGEMENT
  • In case of burns apply shatadhouta gritam to the part.
  • Sneezing and rhinitis- Talam with Rasnadi choorna and Jambeera swarasa, dhoomapana with Haridra dhooma varti .
