INTRA UTERINE UTTARABASTI

INTRODUCTION

The procedure in which the medicine is administered through intra uterine route.

MATERIALS REQUIRED
  • Ref General Requirements SOP
  • Instruments – Sponge holding forceps, Cuscus/Sims Speculum, Uterine sound.
  • Basti netra – IV Canula/ Infant feeding tube/ IUI canula.
  • Basti putaka – 10 cc Syringe.
  • Medicine – Oil/ Ghee
  • Xylocaine Gel
  • Normal saline
  • Surgical gloves – 2
  • Hole towel – 1
  • Gauze pieces – 4
  • Cotton
  • Bowl – 1
  • Kidney tray -1

    Manpower

  • Physician -1

  • Attendant – 1

PREPARATION

 Preparation of tray

  • Take a clean tray.
  • Put medicine and all other required materials for sterilization in this tray.

Preparation of patient

  • Check vitals.
  • Vegotsarga
PURVA KARMA
  • The medicine for uttarabasti is taken in a pre-cleaned container.
  • The medicine, along with all the other required materials is subjected to sterilisation.
  • The autoclaved medicine is taken in 10 cc syringe and attached to infant feeding tube after wearing surgical gloves.
  • Pulse and B.P. are recorded.
PRADHANA KARMA
  • The patient is asked to lie in lithotomy position.
  • The genital area is cleaned from inner to outer vestibule to both thigh region with normal saline using sponge holding forceps.
  • The hole towel is placed exposing the genital region.
  • P/V examination is done to assess the size, shape and position of uterus.
  • Cuscus speculum is lubricated with xylocaine and gently inserted.
  • Uterine sound is then inserted to dilate the external os and to know the length and direction of cervix.
  • Infant feeding tube is introduced slowly through cervical opening and the medicine filled in the syringe is gently pushed into the uterine cavity.
  • Once the administration of medicine has been completed, the infant feeding tube and cuscus speculum is removed slowly.
PASCHAT KARMA
  • The patient is asked to lie down in supine position and asked to take rest.
DURATION
  • 30 – 45 minutes
INDICATIONS

 

  • Female infertility
  • Menstrual problems like Menorrhagia, Amenorrhea, Dysmenorrhea
  • Non – expulsion of placenta
  • Fallopian tube blockage
  • Uterine fibroid
  • Cervical erosions
  • Endometriosis
  • Cervical Polyp
  • PCOD
CONTRAINDICATIONS
  • Hypersensitivity
  • Carcinoma cervix
  • Virginity
  • Bleeding disorders
  • Carcinoma of the bladder
PRECAUTIONS
  • Uttarabasti procedure should be carried out by an expert physician under all aseptic precautions.
  • The medicine for uttarabasti along with all the required materials should be autoclaved.
  • Basti netra should be insert carefully without shaking.
  • Basti Netra should not be inserted beyond and lesser than the prescribed limit.
  • Urge of urine or motion neither be ignored nor forced.
  • Avoid spicy food and any kind of sexual activity.
COMPLICATIONS AND MANAGEMENT
  • Lower abdominal pain: it is usually self-limiting and if necessary give fomentation with hot water bag and antispasmodics can be given.
  • Infection: vaginal douche with triphala kashya. This condition may require a course of antibiotic therapy.
  • Per vaginal bleeding: It was considered as a complication, only if it is very much disturbing the patient.
  • Retention of basti dravya: insert sodhana dravya siddha phala varti in yonimarga.
