KASHAYA DHARA

INTRODUCTION

Kashaya Dhara is a type of fomentation where hot medicated decoction prepared is poured over the body in a continuous and constant stream for specific duration of time.

MATERIALS REQUIRED
  • Ref General Requirements SOP
  • Prepared Kashaya dravya – 5 L
  • Oil for abhyanga – 100 ML
  • Thalam
  • Galanthika – 2
  • Bowl – 1
  • Vessel – 4
  • Cotton balls – 2
  • Towel – 1
  • Hot water bath

PREPARATION

Preparation of medicine

  • 800 gm of drug is boiled in 16 litres of water & reduced to ¼ th quantity.

 Preparation of Thalam

  • Prescribed drugs taken and thalam prepared.
  • Refer video and SOP of Thalam

Preparation of tray

  • Take a clean tray.
  • Put Kashaya in a vessel, langotti, towel, thalam, oil for abhyanga in bowl, galanthika.

Preparation of patient

  • Check vitals.
  • Jeernaharalakshana assessed.
  • Vegotsarga
  • Make patient wear langotti.
PURVA KARMA
  • The patient is asked to sit comfortably over the dhroni.
  • Ear plugs are inserted properly into both ears.
  • Rasnaditalam is placed over the vertex.
  • Then the patient is asked to sit with legs extended
  • The body and foot is cleaned with a towel.
  • Initially Sarvanga abhyanga is done in all 7 postures starting with Sitting, Supine, Right Lateral, Supine, Left Lateral, Supine and Sitting .

 

PRADHANA KARMA
  • The procedure done in all 7 postures starting with Sitting, Supine, Right Lateral, Supine, Left Lateral, Supine and Sitting .
  • Prepared Kashaya is sufficiently heated in avessel.
  • Lukewarm Kashaya is taken in galanthika and is poured on to the either side of the patient’s body in a synchronised manner from a height of 12 angula.
  • Temperature of the Kashaya is checked by pouring it over the dorsum of hand of therapist and confirmed with the patient by pouring it over his foot.
  • In sitting posture luke warm Kashaya is poured over the body covering the neck region to hands, neck to lower back, hip to foot and neck to foot region in synchronized manner .
  • The poured Kashaya is collected from droni in another vessel and added to the heating vessel.
  • The temperature of the Kashaya is maintained throughout the procedure by reheating it.
  • In each posture the dhara is continued until the body of patient is sufficiently fomented
  • The patient is made to line in supine posture
  • The Kashaya, sufficiently heated is poured over the abdomen to hands and hip to foot. Then the entire body is covered by pouring it in a linear pattern in a continuous and constant stream from hands to feet.
  • The temperature of the Kashaya is maintained throughout the procedure by reheating it.
  • The Kashaya is poured in circular pattern over the joints.
  • The patient is then asked to lie in left lateral position with right knee flexed.
  • The warm Kashaya is poured over the hand to hip by one therapist, while the other therapist covers back region to foot.
  • Then the patient is asked to lie in supine position and a same sequence of pouring the warm Kashaya is repeated.
  • The patient is then asked to lie in right lateral position with left knee flexed.
  • The heated Kashaya is poured over the hand to hip by one therapist, while the other therapist covers back region to foot.
  • The patient is again asked to lie in supine position and same sequence of pouring warm Kashaya is repeated on the patient’s body.
  • Finally, the patient is made to sit with legs extended and respective pattern of pouring Kashaya mentioned for sitting posture are repeated.
PASCHAT KARMA
  • The earplugs and thalam are removed.
  • The patient’s body is cleaned with a towel.
  • Patient is asked to take hot shower after resting for a while.
  • Rasnadichurna is applied over vertex after bath.
DURATION
  • 45 minutes – 1 hour

CONTRAINDICATIONS
  • Pregnancy
  • Weak persons
  • Fever
  • Cardiac Problem
  • Indigestion
  • Sinusitis
  • Hemorrhagic Disorders
  • Extreme Fatigue
PRECAUTIONS
  • Should closely observe the general condition of the patient during the procedure.
  • It must be ensured that the temperature of Kashaya is maintained throughout the procedure.
  • Avoid the Kashaya from falling into the eyes and ears.
  • Should maintain the 12 angula distance while pouring the medicine.
  • Stream should be uniform and continuous.
COMPLICATIONS AND MANAGEMENT
  • Hypotension, Giddiness and Fatigue: sprinkle cold water over the face
  • Fainting: Sprinkle cold water and drakshadi Kashaya can be given internally.
  • Burns: Apply ShathadoudhaGritha or MahathikthakaGritha
