STANDARD OPERATING PROCEDURES

PIZHICHIL

INTRODUCTION

Pizhichil is a type of fomentation where hot  medicated oil is poured over the body in a continuous and constant stream for a specific duration of time.

MATERIALS REQUIRED
  • Ref General Requirements SOP
  • Taila for Pizhichil – 4 L
  • Thalam
  • Cotton cloth – 4 pieces or Galanthika – 4
  • Vessel – 4
  • Cotton balls – 2
  • Towel – 2
  • Hot water bath
  • Dharatable – 1
PREPARATION

Preparation of Thalam

  • Prescribed drugs taken and thalam prepared.
  • Refer video and SOP of Thalam

Preparation of tray

  • Take a clean tray.
  • Puttaila in a vessel, langotti, towel, thalam, cotton cloth / galanthika.

Preparation of patient

  • Check vitals.
  • Jeernaharalakshana assessed.
  • Vegotsarga
  • Make patient wear langotti.
PURVA KARMA
  • The patient is asked to sit comfortably over the droni.
  • Ear plugs are inserted properly into both ears.
  • Rasnaditalam is placed over the vertex.
  • Then the patient is asked to sit with legs extended
  • The body and foot is cleaned with a towel.
PRADHANA KARMA
  • The procedure is done in all 7 postures starting with Sitting, Supine, Left Lateral, Supine, Right Lateral, Supine and Sitting .
  • The taila for pizhichil is sufficiently heated using a hot water bath.
  • Temperature of the taila is checked by pouring it over the dorsum of hand of the therapist and confirmed with the patient by pouring it over his foot.
  • Lukewarm taila is poured on either side of the patient’s body by squeezinga piece of cloth that is periodically dipped in warm oil in a synchronised manner from a height of 12 angula.
  • While pouring the taila, massage is done simultaneously using the other hand.
  • In sitting position,Pizhichil is done in linear pattern over the neck region to the hands, neck to lower back,hip to foot and neck to foot.
  • The poured Taila is collected from droni in another vessel and added to the heating vessel.
  • The temperature of the taila is maintained throughout the procedure by reheating it in hot water bath.
  • Then the patient is asked to lie in supine position.
  • Pizhichil is done over the abdomen to hands and hip to foot. Then the entire body is covered by pouring the warm taila by squeezing the pieces of cloth soaked in the same in a linear pattern from hands to feet.
  • The Taila is poured in circular pattern over the joints.
  • The patient is then asked to lie in left lateral position with right knee flexed.
  • The warm Taila is poured over the hand to hip by one therapist, while the other therapist covers back region to hip. At the same time the other two therapist cover the hip to foot area.
  • Then the patient is asked to lie in supine position and the same sequence of pouring the warm Taila is repeated.
  • The patient is then asked to lie in right lateral position with left knee flexed.
  • The warm Taila is poured over the hand to hip by one therapist, while the other therapist covers back region to hip. At the same time the other two therapist cover the hip to foot region.
  • Then the patient is again asked to lie in supine position and same sequence of pouring warm Taila is repeated.
  • Finally, the patient is made to sit with legs extended and respective pattern of pouring Taila mentioned for sitting posture are repeated.
PASCHAT KARMA
  • The earplugs and thalam are removed.
  • The patient’s body is cleaned with a towel.
  • Patient is asked to take hot shower after resting for a while.
  • Rasnadichurna is applied over vertex after bath.
DURATION
  • 45 minutes – 1 hour
INDICATIONS
  • Hemiplegia
  • Paraplegia
  • Peripheral neuropathy
  • Degenerative conditions
  • Muscular and ligamental injuries
  • Fracture / dislocation of joints
  • Joint stiffness
  • Healthy person for rejuvenation
CONTRAINDICATIONS
  • Pregnancy
  • Fever
  • Cardiac Problem
  • Indigestion
  • Sinusitis
  • Obesity
PRECAUTIONS
  • Should closely observe the general condition of the patient during the procedure.
  • It must be ensured that the temperature of taila is maintained throughout the procedure.
  • Avoid the taila from falling into the eyes and ears.
  • Should maintain the 12 angula distance while pouring the medicine.
  • Stream should be uniform and continuous.
COMPLICATIONS AND MANAGEMENT
  • Hypotension, Giddiness and Fatigue: sprinkle cold water over the face.
  • Fainting: Sprinkle cold water and drakshadi Kashaya can be given internally.
  • Burns: Apply ShathadoudhaGritha or MahathikthakaGritha
