STANDARD OPERATING PROCEDURES

UPANAHAM

INTRODUCTION

The procedure of applying warm herbal paste to the diseased parts of the body followed by bandaging is referred by the name Upanaha Sveda.

MATERIALS REQUIRED
  • Ref General Requirements SOP
  • Fine powder (Nagaradi Choornam / Kottanchukkadi Choornam / Jadamayadi Choornam etc according to Dosha pradhanyatha)- 50 – 100g
  • Leaves of Eranda, Arka – To cover the area
  • Drava dravya-100ml
  • Amla dravya (Dhanyamla / Kanji / Takra)- 50 – 100ml
  • Godhuma-30-60g
  • Oil for Upanaham-50 ml
  • Saindhava- 20 – 30g
  • Bandage cloth (15cm x 2m) – 1
  • Scissor-1
  • Vessels – For preparing the paste

PREPARATION

Preparation of tray

  • Take a clean tray.
  • Put towel,  preapared paste, scissor, eranda/arka, bandage cloth etc.

Preparation of patient

  • Check vitals.
  • Jeernahara lakshana assessed.
  • Vegotsarga
  • Make patient wear langotti.
PURVA KARMA
  • The required quantity of choorna is taken in a clean vessel.
  • To the choorna first Godhuma is added.
  • Then saindhava is added.
  • All the powders are mixed thoroughly to form a uniform mixture.
  • The Drava dravya used to mix the powder is heated and poured in to the vessel slowly mixing the choornas.
  • Avoid lumps while mixing the choorna.
  • The mixture is then heated in low flame to cook it evenly.
  • Once the mixture become a thick paste it is removed from stove .
  • Taila is added to the paste and mixed properly.
  • Amla dravya if any should be added after heating the paste
PRADHANA KARMA
  • The patient is made to sit or lie comfortably in a cot.
  • The tender region is palpated.
  • The prepared lukewarm paste for upanaha is applied over the painful region evenly. 
  • The paste is  applied in the opposite direction of hair follicles.
  • Thickness of the paste should be half inch.
  • The paste is covered using vathahara patras.
  • It is then bandaged using Gauze cloth to remain tightly for 12 hours.
PASCHAT KARMA
  • The upanaha tied at day time should be removed during night time and upanaha tied at night time should be removed during the day time.
  • The bandage and the herbal paste are removed and the area is properly cleaned with hot water.

CONTRAINDICATIONS
  • Infections
  • Wounds
PRECAUTIONS
  • Consistency of the herbal paste – Thickness should be 2 – 5 mm.
  • Thin application is likely to get dry early.
  • If it is thicker, it causes irritation of the skin precipitating skin eruptions and itching.
  • The bandage should not be too tight or too loose.
  • Avoid burning sensation of the area by removing the paste at proper time.
COMPLICATIONS AND MANAGEMENT

Redness, discolorations, burning sensations –

  • Stop the procedure immediately.
  • Wash the area with cold water.
  • Local application with drugs like Nalpamaradi kera / Eladi kera is advised.
  • The treatment can be restarted within few days.
